Special Olympics Ontario: Oakville Rhythmic Gymnastics

Registration 2009-2010

The fee this year is again $60.00, this also covers the cost of registration in Gymnastics Ontario. Please write cheque for $60.00, made out to "SOO Oakville Butterflies", and if possible submit it with your paperwork.

This is a summary of the forms which will be required to register for this season. Please keep in mind that ALL required forms must be submitted in order for the athlete to participate. You may click on the links to download each form as a pdf.

Athletes

Required for All Athletes:

  1. Athlete Profile
    • Returning Athletes will receive a partially completed form to finish.
    • New Athletes may download a copy of the Athlete Profile form.
  2. Rules And Expectations
    • This must be read and signed by the athlete and the athlete's caregiver.
  3. Gymnastics Ontario Registration (and yes we have to do this every year.)
    • You do not need a club number; for the club name put "SOO Oakville Butterflies"

New Athletes also Require:

  1. Special Olympics Ontario Athlete registration form
  2. If the athlete has Down's Syndrome the Atlanto-axial form is also required
Volunteers

Required for ALL Coaches

  1. Gymnastics Ontario Registration (see note above)

Required for NEW Coaches and other Volunteers

  1. Special Olympics Ontario Volunteer registration form
  2. A Police check is required. (more information coming soon)
