Registration 2011-2012

The fee this year is $75.00o. Please write cheque for $75.00, made out to "SOO Oakville Butterflies", and if possible submit it with your paperwork.

This is a summary of the forms which will be required to register for this season. Please keep in mind that ALL required forms must be submitted in order for the athlete to participate. You may click on the links to download each form as a pdf.

Athletes	Required for All Athletes: Athlete Profile Returning Athletes will receive a partially completed form to finish. New Athletes may download a copy of the Athlete Information & Medical History Form. Rules And Expectations This must be read and signed by the athlete and the athlete's caregiver. New Athletes also Require: Special Olympics Ontario Athlete registration form If the athlete has Down's Syndrome the Atlanto-axial form is also required
Volunteers	Required for NEW Coaches and other Volunteers Special Olympics Ontario Volunteer Registration Form A Police check is required. (See Debbie or MaryAnn)
